Fill out and submit to the Principal; these forms fall under privacy provisions for the student so copying is not recommended.
This form is used toward progressive discipline but can also be used to create a case for additional supports/resources, workshops (eg. equity training) etc.
The principal must investigate all reports submitted by board employees, as outlined in subsection 300.2(3) of the Education Act; and must provide the Safe and Caring Schools Incident Reporting Form – Part II Acknowledgement of Receipt of Report to the teacher who submitted the form.
Once the investigation is complete, the principal must communicate the results of the investigation to the teacher who made the report in a timely manner.
Communication between the principal and school staff about the investigation and the results of the investigation is a shared responsibility, and is an important factor in meeting student needs and fostering collaboration in the school, and helping prevent future inappropriate behaviour from taking place (From Ministry of Education Policy/Program Memorandum 145).
A best practice is for the Principal to maintain these forms for tracking student behaviour and progressive discipline
